A stiff neck is typically characterized by soreness and difficulty moving the neck, especially when trying to turn the head to the side. A stiff neck may also be accompanied by a headache, neck pain, shoulder pain and/or arm pain, and cause the individual to turn the entire body as opposed to the neck when trying to look sideways or backward. Stiff Neck Causes and Symptoms The most common causes of a stiff neck include the following: By far the most common cause of a stiff neck is a muscle strain or soft tissue sprain, and in particular the levator scapula muscle is susceptible to injury. Located at the back and side of the neck, the levator scapula muscle connects the cervical spine (the neck) with the shoulder. The levator scapula muscle may be strained throughout the course of many common, everyday activities, such as: Sports injuries that strain the neck, such as a fall or sudden impact that pushed the head to the side Any activity that involves repeatedly turning the head from side to side, such as swimming the front crawl stroke Poor posture, such as slouching while viewing the computer monitor or looking downward at a mobile phone for prolonged periods Excessive stress, which can lead to tension in the neck Holding the neck in an abnormal position for a long period, such as cradling a phone between the neck and shoulder. Sleeping with the neck at an awkward position that strains the neck muscles

The shoulder is a ball and socket joint, made up of three bones, the humerus (upper arm bone), the scapula (shoulder blade) and the clavicle (collar bone). The ball at the top end of the humerus (humeral head) fits into the socket (glenoid) of the scapula to form the shoulder joint. The humeral head and glenoid are covered with cartilage, which allows for smooth joint motion. The shoulder has the greatest range of motion of any joint in the body, allowing you to position your hand anywhere in space. The coordinated activity of numerous muscles working together in set patterns is required to produce this motion. The rotator cuff is a group of muscles closely wrapped around the top of your arm bone (humerus). These muscles originate from your shoulder blade and their tendons form a cuff which covers the joint. The rotator cuff muscles are important to normal shoulder function. Together they act to stabilize and rotate the shoulder joint. In the Arthritic shoulder the normal cartilage (smooth surface of joint) is worn away and there is bone-on-bone without the normal smooth gliding surfaces, which are able to glide on one another with little friction and wear. The joint may also become irregular from boney growth (osteophytes), which is the body's attempt to "heal" the cartilage injury. Pain is usually due to the irregular joint surfaces rubbing on one another and from the inflammation of this wear and tear. If you can't lift your arm, perform everyday tasks or sleep at night, your doctor may recommend arthroplasty, the medical name for a shoulder replacement operation. The surgeon removes the damaged parts of the shoulder and replaces them with artificial parts. The surgeon can replace either just the head of the humerus bone, the ball, or replace both the ball and the socket, the glenoid.

A shoulder replacement is a less common procedure unlike a knee replacement or a hip replacement procedure. The shoulder is a ball-and-socket joint which has a wider range of motion than any other joint in the human body. Shoulder joint gives strength and range of motion to the arm. The shoulder joint is made of a rounded end of the upper arm bone (head of the humerus) which glides against the small socket (glenoid) in the shoulder blade (scapula). Muscles and tendons, such as the rotator cuff, help hold the joint in place. The surfaces of the joints are normally covered with smooth cartilage. Over a period of time, the continuous wear and tear of the joint may lead to loss of the cartilage and mechanical deterioration of the joint. The result can be pain. You can have a stiff shoulder that grinds or clunks. This can lead to a loss of strength, decreased range of motion in the shoulder and impaired function. This is called Osteoarthritis or wear-and-tear arthritis. Osteoarthritis is a degenerative joint disease and is one of the most common diseases which is treated by a shoulder replacement surgery when all other conservative treatments like medications, physical therapy, activity modification etc. fail. Osteoarthritis generally affects people above 50 years of age or those people who have a history of shoulder injury. Diagnosis Your doctor will do a physical examination and take your medical history. After this is done, a series of standard x-rays are taken. This helps to know the condition of the shoulder. An X-ray of the shoulder would show the following Loss of the normal cartilage joint spaceIrregularity in the shape of the boneBone spursLoose pieces of bone and cartilage floating inside the joint A CT scan and Magnetic Resonance Imaging (MRI) may also be necessary to evaluate bone quality as well as the condition of important surrounding structures such as the rotator cuff tendon Treatment Options 1) Shoulder ArthroscopyShoulder arthroscopy is a minimally invasive procedure. It is performed to diagnose and treat many shoulder injuries and diseases without a major cut. This procedure is performed by making small incisions through which a small camera, called an arthroscope is inserted. The doctor sees the joint and the surrounding areas on a computer screen. This helps in examining and performing surgery on the bones and soft tissues in and around the shoulder. Most surgeons prefer arthroscopy instead of open shoulder surgery because an arthroscopic procedure has many benefits. Some of the major advantages of this procedure are:Quicker recovery and shorter hospital stay Less blood lossLess scarringLess risk of infection2) Shoulder ReplacementIn a shoulder replacement surgery, the shoulder joint is replaced by an implant or prosthesis. Shoulder replacement involves replacing the humeral head and the glenoid with artificial components. The humeral head replacement is generally made from a metal alloy, while the glenoid component is made from polyethylene plastic. The new components may be anchored by cement or press-fit into place so that the bone grows in around them. In a shoulder replacement surgery a three- to four-inch incision is made along the space between the arm and the collarbone. The procedure lasts about an hour or so. The incision is then closed with staples or stitches. 3) Shoulder ResurfacingShoulder resurfacing surgery is an advanced technique is an excellent alternative to total shoulder replacement surgery. This is a bone conserving method since major portion of the bone is not cut unlike a shoulder replacement surgery. Shoulder resurfacing surgery has some advantages

Similar to the hip joint, the shoulder joint is a ball-and-socket joint that allows for a wide range of motion. Though less common than hip replacements and knee replacements, shoulder replacement surgery can be just as effective at relieving severe pain from arthritis of the shoulder. In shoulder replacement surgery, the damaged parts of the shoulder are removed and replaced with artificial components, called a prosthesis. The treatment options are either replacement of just the head of the humerus bone (ball), or replacement of both the ball and the socket (glenoid).

The shoulder joint is very complex and involves three bones and more than one joint. These bones are the clavicle (collar bone), the scapula (shoulder blade), and the humerus (upper arm bone). Numerous muscles, ligaments, and tendons surround the joint. The upper end of the arm bone (humerus) and the outside edge of the scapula bone (glenoid) form a ball-and-socket joint . This joint is remarkable because it typically allows greater range of motion than any other joint in your body. When Is A Shoulder Replacement Needed? Shoulder replacement surgery is most commonly used for severe degenerative joint disease (osteoarthritis) or rheumatoid arthritis of the ball-and-socket joint. When the smooth surfaces (cartilage) of the ball and socket become rough, they rub against each other rather than glide. This rubbing causes pain and stiffness that limits daily activities, as well as interferes with sleep. Other indications that justifies shoulder replacement are severely communited fracture of proximal humerus which cannot be reconstructed and avascular necrosis of humeral head. A shoulder replacement is performed to alleviate shoulder pain. It also helps to improve the range of motion of your shoulder joint, which also improves your function and the quality of your life. What Are The Types Of Replacement? The most common types of shoulder replacement are: Total shoulder replacement: Here, the ball (humeral head) of the shoulder joint is replaced with an implant that includes a stem with a smooth, rounded metal head. The socket (glenoid) is replaced with a smooth, specialized plastic that is cemented into place. Hemireplacement: Where only the ball (humeral head) is replaced with metal stem and head. Reverse total shoulder replacement: Here, the normal structure of the shoulder is reversed. The ball portion of the implant is attached to the scapula (where the socket normally is) and the artificial socket is attached to the humeral head (where the ball normally is). This allows the stronger deltoid muscles of the shoulder to take over much of the work of moving the shoulder, increasing joint stability. A reversed procedure is use for patients with a severely torn and compromised rotator cuff. It is also commonly used in revision surgery cases.

With increasing life span of our population is increasing the shoulder joint also undergoes degeneration like the hip and the knee joint. Osteoarthritis of the Shoulder is a very common problem in the elderly. State of the art shoulder prosthesis is now available which can be implanted in selected patients. This prosthesis gives very satisfactory pain relief. The patients who undergo such surgery regain a lot of their shoulder movement back. Shoulder replacement is also a good alternative in treating patients with fracture of the upper end of the arm bone (Humerus). In the elderly patients the bones of the arm are so weak that operative fixation often fails. A replacement of the shoulder joint gives early rehabilitation and pain relief REVERSE SHOULDERSome patients with arthritis of the shoulder also have poor strength in a specific group of muscles called rotator cuff. In these patients a regular total shoulder replacement does not give the desired shoulder movement. A reverse shoulder is a new type of prosthesis, which is useful in these patients helping them to regain functional range of movement. Reverse total shoulder replacement arthroplasty enables experienced shoulder surgeons to treat patients with conditions that previously had no solution. These conditions include rotator cuff tear arthroplasty instability with anterosuperior escape pseudoparalysis and failures of surgery for arthritis and facture management. After performing a clinical exam a shoulder surgeon experienced with rotator cuff tear arthropathy can suggest what type of surgery is most likely to be helpful to the individual with the condition. Individuals are most likely to benefit from this surgery if they are well motivated and in good health. Rotator cuff tear arthropathy is a complex shoulder problem that occurs when there is a large tear of the rotator cuff tendons that causes arthritis in the glenohumeral (shoulder) joint. In a normal shoulder, the rotator cuff tendons hold the humerus (ball) centered in the glenoid (socket). When there is a large tear, the humerus migrates upward and there is loss of cartilage. In this condition, a stan dard rotator cuff repair will not be effective because it will not help the arthritis, and a traditional shoulder replacement will not be effective because the shoulder will continue to be in a position upward out of the joint.

Shoulder replacement surgery involves replacing the shoulder joint if it has been damaged or worn away, usually by arthritis or injury. The primary goal of shoulder replacement surgery is pain relief, with a secondary benefit of restoring motion, strength, function, and assisting with returning patients to an activity level as near to normal as possible. Causes Shoulder replacement surgery is usually done when there is severe pain in the shoulder area, which limits the ability to move the arm. Common reasons behind shoulder pain are: Osteoarthritis Poor result from a previous shoulder surgery Rheumatoid arthritis Badly broken bone in the arm near the shoulder

Shoulder replacement is a surgical procedure in which all or part of the glenohumeral joint is replaced by a prosthetic implant. Shoulder joint replacement surgery generally is conducted to relief arthritis pain or fix severe physical joint damage. Shoulder replacement surgery is an option for treatment of severe arthritis of the shoulder joint. Arthritis is a condition that affects the cartilage of the joints. As the cartilage lining wears away, the protective lining between the bones is lost. When this happens, painful bone-on-bone arthritis develops. Severe shoulder arthritis is quite painful, and can cause restriction of motion. While this may be tolerated with some medications and lifestyle adjustments, there may come a time when surgical treatment is necessary.

AnatomyShoulder is made up of three bones, upper arm bone (humerus), shoulder blade (scapula), and collarbone (clavicle). The shoulder is a ball-and-socket joint: The ball, or head, of your upper arm bone fits into a shallow socket in your shoulder blade. This socket is called the glenoid. The surfaces of the bones where they touch are covered with articular cartilage, a smooth substance that protects the bones and enables them to move easily. A thin, smooth tissue called synovial membrane covers all remaining surfaces inside the shoulder joint. The muscles and tendons that surround the shoulder provide stability and support.Conditions for shoulder replacementOsteoarthritis (Degenerative Joint Disease)This is an age-related "wear and tear" type of arthritis. It usually occurs in people 50 years of age and older, but may occur in younger people, too. The cartilage that cushions the bones of the shoulder softens and wears away. The bones then rub against one another. Over time, the shoulder joint slowly becomes stiff and painful.Rheumatoid Arthritis : This is a disease in which the synovial membrane that surrounds the joint becomes inflamed and thickened. This chronic inflammation can damage the cartilage and eventually cause cartilage loss, pain, and stiffness. Rheumatoid arthritis is the most common form of a group of disorders termed "inflammatory arthritis."Post-traumatic Arthritis: This can follow a serious shoulder injury. Fractures of the bones that make up the shoulder or tears of the shoulder tendons or ligaments may damage the articular cartilage over time. This causes shoulder pain and limits shoulder function.Rotator Cuff Tear Arthropathy A patient with a very large, long-standing rotator cuff tear may develop cuff tear arthropathy. In this condition, the changes in the shoulder joint due to the rotator cuff tear may lead to arthritis and destruction of the joint cartilage.Avascular Necrosis (Osteonecrosis): Avascular necrosis is a painful condition that occurs when the blood supply to the bone is disrupted. Because bone cells die without a blood supply, osteonecrosis can ultimately cause destruction of the shoulder joint and lead to arthritis.Severe Fractures: A severe fracture of the shoulder is another common reason people have shoulder replacements. When the head of the upper arm bone is shattered, it may be very difficult to put the pieces of bone back in place.Failed Previous Shoulder Replacement Surgery:-Although uncommon, some shoulder replacements fail, most often because of implant loosening, wear, infection, and dislocation. When this occurs, a second joint replacement surgery -called a revision surgery -may be necessary.Indications for Shoulder Joint Replacement : People who benefit from surgery often have :Severe shoulder pain that interferes with everyday activities, such as, dressing, toileting, and washing.Moderate to severe pain while resting. This pain may be severe enough to prevent a good night's sleep.Loss of motion and/or weakness in the shoulder.Failure to substantially improve with other treatments such as anti-inflammatory medications, cortisone injections, or physical therapy.X-rays or MRI shows arthritis or other problems.Total shoulder replacement implant :The humeral implant consists of a metal ball that replaces the head of the humerus, and a stem that is secured into the main arm bone (humerus). The humeral stem is made of titanium for maximum strength. The head is made of cobalt chrome to provide a smooth surface for movement with the glenoid component, which is made of polyethylene. The metal ball and stem are selected by the surgeon from multiple sizes to fit the contour and shape of each patient's humerus. This two-piece construction is known as a modular implant.

The shoulder joint can be replaced by an artificial shoulder joint for pain caused by arthritis or when the shoulder is severely fractured or broken. Arthritis is when the cartilage on the ends of the bones is gone and there is bone rubbing on bone in the joint. Arthritis can be caused by fractures, rheumatoid disease, torn rotator cuff tendons or just by wear and tear over time. The shoulder is a ball and socket joint. The ball is the upper part of the arm bone (humerus) and is called the humeral head. The socket is smaller and is part of the shoulder blade (scapula). The ball is held in the socket by ligaments and movement is controlled by the rotator cuff muscles. The rotator cuff muscles start on the shoulder blade and turn into tendons which attach to the bone below the ball. In shoulder replacement surgery the ball is replaced with a metal ball attached to a stem. The stem is inserted down the shaft of the humerus. Sometimes cement is utilized to keep the stem in place. The socket sometimes is replaced with a plastic piece which is usually fixed to a groove in the socket with cement. Whether a socket is used or not depends upon how bad the arthritis is in the shoulder and whether the rotator cuff tendons are intact.
